Cosmo is best understood as a healer survivalist.
His active ability is Sharing is Caring.
Cosmo is a dedicated supporter whose active ability, Sharing is Caring, lets him sacrifice one of his own Hearts to heal a targeted teammate. As one of only four Healers currently in the game, this capability is invaluable when allies are injured or at critical risk. A hidden benefit of this ability is its secondary effect: when targeting a teammate, Cosmo can see their silhouette highlighted through walls along with their current animation state. Because the ability has extremely long range, this essentially turns Cosmo into a team-wide radar, letting him track distractors and players in danger from great distances. Be cautious about over-healing, though, as donating too many Hearts puts Cosmo himself in jeopardy and may force teammates to divert resources to save him instead.
Cosmo's glaring weakness is his 1-star Skill Check. The tiny check window makes consistent extraction difficult, and even though he extracts faster than Bassie, Blot, and Shrimpo, all four share that same 1-star handicap. To manage this, equip Skill Check trinkets such as Mime Makeup (removes the penalty for missing), Thinking Cap (enlarges the check window), or Blue Bandana (helpful for newer players, though it slightly slows progress). If the player has no issues with small Skill Checks, Magnifying Glass or Participation Award can speed up Machine progress, but beware that lag or dropped frames make these risky choices. Vee's Remote is another strong option, instantly completing Machines in dangerous dead ends or open areas where lingering is risky.
For survivability, Pink Bow is a top-tier first trinket, pushing Cosmo's run speed to 26.9 so he outruns all Twisteds before Panic Mode except Twisted Dyle. Pair it with Speedy Shoes for 28.2 run speed, or Vanity Mirror to outrun every Twisted except Twisted Dyle even during Panic Mode. Savory Charm provides a one-time safety net when Cosmo drops to 1 Heart, which happens frequently given his healing playstyle. Train Whistle helps him avoid losing Hearts during Ichor Leaks or when Twisted Vee is on the Floor.
Cosmo's above-average Stealth makes him especially effective at healing distractors in the field, since distracted Twisteds will not re-target unless they round a corner. He is a natural fit for rounds with multiple Main Character Toons, as his ability stretches the value of Health Kits twice as far for them. When looking for items, prioritize Health Kits and Bandages so you can use your ability more frequently; ask teammates to alert you when they spot healing items so you can redistribute Hearts to distractors or stockpile for later Floors. Since healing items are rare, also grab Skill Check Candies and Stopwatches to ease extraction, and spend the extra Tapes on healing items from Dandy's shop. If a non-Main teammate sits at 1 Heart, let them take a Health Kit directly since it fully restores them, whereas your heal only provides 1 Heart.
For team synergy, Cosmo pairs exceptionally well with Ginger, who can heal him back while he is low, creating an effective infinite healing loop, though this requires significant Tape income for Ginger. He also synergizes naturally with Sprout, keeping Sprout alive longer and sharing Hearts through his faster cooldown cycle. If Bassie is in the group, coordinate healing duties: let Bassie hold most healing items and heal Cosmo when he is low, and have Bassie deposit items on the floor when Twisted Gigi spawns to prevent theft. For ability-focused trinket loadouts, Ghost Snakes In A Can reduces the Sharing is Caring cooldown to 55 seconds, and stacking Sweet Charm on top brings it further down to about 50.2 seconds. Memory Locket highlights healing items on the floor through walls, which is especially useful in unorganized public runs where heals get snatched quickly. Dandy Plush provides passive Tape income so Cosmo can afford shop healing items without competing with Tape-hungry teammates like Ginger.

Cosmo is a helpful yet cautious Toon whose sweet and caring nature is evident the moment you purchase his Toon License. True to Dandy's description, he frequently gives out homemade sweets to fellow Toons to cheer them up and offers heartfelt compliments when he senses they need encouragement. His hobby of baking appears to stem from both a personal source of comfort and a genuine desire to help others. However, Cosmo also carries a notable streak of anxiety; his nervousness surfaces in his dialogue whenever he finishes a Machine or descends to lower Floors. He can be clueless about jokes and somewhat gullible to teasing, as seen in his interactions with Finn and Gigi, but he always makes sure to care for his friends when they need it most. Importantly, despite his soft-spoken demeanor, Cosmo is no pushover. He firmly refuses Rodger's attempts to pry into personal details about his best friend Sprout, and he tells Eggson outright that he cannot hide the eggs for him to find.
