Upon use, Air Horn drastically decreases Stealth by 50 for 10 seconds and simultaneously alerts nearby and specific Twisteds to your current location. The Stealth reduction is a flat value rather than a percentage, which means Air Horn consistently pulls Twisted attention regardless of your Toon's base Stealth stat. While the effect is active, Twisteds that would normally patrol or chase other targets may redirect toward you, making the Air Horn one of the most powerful aggro-drawing tools in the game. Air Horn can be found on Floors as a random spawn or purchased from Dandy's Shop for 55 Tapes. With the Dandy Plush trinket the price drops to 33 Tapes, with the Frugal Card to 49 Tapes, and with both to just 28 Tapes. Using any Rare item rewards the player with 5 Tapes upon consumption. The Air Horn produces a distinctive sped-up sound effect when activated, making it audible to all players in the vicinity.
How Stealth Reduction Works

Air Horn's flat reduction, by contrast, simply subtracts 50 from your current Stealth value, making it equally effective on Toons with high or low base Stealth. Note that Shrimpo's Passive causes him to have negative Stealth at all times, so using any Stealth-modifying item on him will produce inverted results. Similarly, percentage-based Stealth items are useless for Brightney and Gourdy because they have 0 Stealth and multiplying 0 by any value still equals 0 — though Air Horn's flat reduction still works on them.

- Air Horns should be given to distractors, such as Pebble, Flutter, Goob, Tisha, Razzle & Dazzle, Looey, Coal, Sprout, and Yatta. These Toons have abilities designed around drawing Twisted attention, so the Air Horn complements their kits naturally.
- Use Air Horns when a teammate is at risk of losing a Heart. Activating it pulls Twisteds toward you and away from the endangered teammate, buying them time to heal or escape.
- On solo runs, Air Horns become potentially dangerous due to attracting Twisteds, leading you to die a bit easier. However, this can be combatted by using them in the Elevator between Floors, where Twisteds cannot reach you but you still earn the 5 Tape reward for consuming a Rare item.
- Be aware that Twisted Gigi can steal items from your inventory, Twisted Bobette can hear items and Tapes being picked up, and Twisted Gourdy requires items from your inventory. Carrying an Air Horn near these Twisteds carries extra risk — consider using it quickly rather than hoarding it.
